People with young onset dementia have different needs from older people with dementia, but age-appropriate services are not always available.
Bromley MindCare, in partnership with City, University of London, is conducting a survey to find out what type of support people with young onset dementia (dementia diagnosed under the age of 65) and their carers would benefit from. We aim to use your feedback to deliver a pilot service suitable for people with young onset dementia in 2021.
Your responses will be kept confidential and your data will be stored securely at City, University of London and not shared outside of the project. To maintain anonymity you will be asked to provide only your initials and date of birth, which will create your ID code. Please put the ID code of the carer and the person with dementia on both surveys.

Please complete both surveys, as one is for the carer and one is for the person with young onset dementia.
